SECTION II. HEALTH GUARANTEE


This guarantee is written for the protection of the puppy, Owner, and Breeder. To the best of Breeder's knowledge, the puppy is in good health and has received all vaccinations and worming as appropriate to the age of the puppy. The puppy will receive a veterinarian check accompanied by a health certificate dated no more than 10 days prior to shipping date. The decision to ship a puppy is that of Owner and Owner’s responsibility of the puppy begins when the puppy is checked in at the airport. Owner should have full knowledge of how to care for the puppy after its arrival and what to look for in case of Hypoglycemia or other conditions that may occur during shipping.
At Owner’s discretion, Owner has 72 hours to get an exam from a licensed veterinarian after receiving the puppy. Breeder will not be responsible for any veterinarian expenses incurred by Owner for any reason. If Owner’s veterinarian finds a life-threatening incurable congenital health problem (excluding common puppy problems) Breeder must be informed within 72 hours after purchase. A written report from the veterinarian must be obtained and provided to Breeder. Breeder’s veterinarian shall be allowed to examine said puppy. Provided that both Breeder’s and Owner’s veterinarians agree that there is a life-threatening incurable health defect which existed at the time of the purchase, and further, provide that the puppy has been properly cared for in Owner’s possession, Breeder guarantees to replace puppy.
Should death occur within the 72 hour period, and autopsy (at Owners’ expense) must be performed at a Diagnostic Laboratory Clinic to determine the cause of death. If the death was caused by a congenital defect the autopsy report must be provided to Breeder proceeding replacement with a new available puppy of the same value, contingent on availability. Owner will be responsible for all shipping expenses, and veterinarian expenses occurred.
Breeder will not be responsible for illness or health problems which are diagnosed after 72 hours of receipt of the puppy by Owner.  No puppy exchanges or refunds will be given by Breeder or its representatives after the initial 72 hours after receipt of puppy by Owner.

HYPOGLYCEMIA AND COCCIDIA
Owner must understand that a toy breed puppy is extremely fragile. Additional care will be needed to ensure the health and well-being of the puppy. Small, young puppies can be prone to both Coccidia and Hypoglycemia which are generally brought on by stress, strenuous exercise, over-handling, or failure to eat a sufficient amount of good quality food. The puppy should continue eating food recommended by Breeder, and any changes in the puppy's diet should be done gradually. Breeder is not responsible for veterinarian bills or replacing puppies when Owners have changed puppy's food radically and puppy becomes ill or dies as a result of the change. Puppies may become depressed when first taken away from mother and littermates. They need to be monitored, encouraged to eat, and force-fed if they refuse to eat. Signs of hypoglycemia are pale, grayish gums instead of bright pink gums, weakness, wobbly gait, and in severe cases, seizures. If seizures occur, Karo Syrup or honey rubbed on a puppy's gums can reverse the condition temporarily until puppy is seen by a licensed vet for emergency treatment. If signs of hypoglycemia occur, get emergency medical treatment immediately. Problems arising from hypoglycemia, malnutrition and dehydration are not covered under the terms of this contract. It is Owner's responsibility to properly care for and observe the condition of the puppy.

 

 

SECTION V. DEPOSIT


Owner understands and agrees that the deposit placed to hold a puppy is non-refundable. A copy of this "Adoption Contract and Health Guarantee" with original signatures must be received by Breeder before the puppy will be shipped. Breeder will provide a health record to Owner at the time Owner takes possession of the puppy.

SECTION VI. STIPULATIONS OF ADOPTION


The person or persons listed as New Owner and whose signature(s) appear on this contract will be the person or persons listed as the new owners on the puppy's registration application.
Owner will provide the puppy with a safe, stable, and comfortable home, proper food, and veterinary care as needed. Puppy will be kept as a house pet, not an "outside" pet. Breeder will gladly accept return of the puppy, at any age, if Owner is ever unable to keep it. However, adoption fee will not be refunded nor shipping fees.
No other provisions, aside from those detailed herein, will be made after the adoption date. The Owner has read the contract, understands the terms, including the section on Hypoglycemia and Coccidia, and acknowledges the receipt of a copy of the contract. Breeder and Owner are not relying on verbal or written statements not contained herein.
This contract is valid for the original Owner who adopted the puppy from Furry Little Angels. If the puppy is transferred to a person other than Owner, this contract is null and void.

SECTION III. BREEDER'S OBLIGATIONS


This puppy is a purebred __________________________________ and is registered with ____________________________________. Upon receipt of registration papers, Owner will also incur an additional registration transfer fee payable to the registry.
Breeder has made every effort to provide you with a healthy puppy; they guarantee that all age appropriate vaccinations and worming are current.  Breeder will estimate size, personality, color, etc. as accurately as possible, but because there are so many variables and factors involved in genetics, Breeder cannot guarantee any of these traits.
At the time of adoption, this puppy is considered to be "Pet Quality"; however is not restricted/limited on registration.  Breeder will not guarantee any breeding potential of the puppy.
Breeder will accept the return of this puppy at any time Owner declines to keep the puppy. However, Breeder is not obligated to refund any, or all, of the adoption fee except in conjunction with Breeder's health guarantee as stated in this contract. Furry Little Angels and its representatives will not reimburse any veterinarian fees incurred by Owner at any time.


SECTION IV. OWNER'S OBLIGATIONS


Owner acknowledges and agrees that he or she is not acting as an agent in the adoption of this puppy and that the puppy will remain solely in his or her custody. Owner further acknowledges and agrees that he or she is not associated with a pet shop or animal broker.
Owner agrees to care for this puppy as would a humane and conscientious pet Owner and agrees to provide the following items:
1. To confine the puppy when outdoors, such as in a secure fenced yard, and refrain from restraining it on a chain or rope. When puppy is outdoors and not in a fenced yard, it should be under the direct supervision of the Owner and on a leash. Harnesses are preferred to collars for leash walking as they offer more security and less pressure on your puppy's neck.
2. To provide quality veterinary care immediately, including but not limited to injuries, illness, and annual check-ups, keeping all vaccinations current according to the American Veterinary Medical Association Guidelines, and kept on heartworm preventative as per veterinarian instructions.
3. To provide adequate housing and proper exercise for this puppy.
4. To provide safe vehicle transportation for the puppy. Owner must not transport the puppy in the open cargo area of a vehicle, such as the back of a pick up truck at any time. The puppy should be restrained with a seatbelt designed for pets or confined to a pet carrier any time it is transported in a vehicle. Please note the danger of placing a pet carrier in the front passenger seat if the vehicle has a passenger seat airbag.
5. To provide the puppy with good quality puppy food until one year of age at which time the puppy should be changed to adult canine food of the same brand. Puppy is eating Purina Pro Plan dry food and supplemented with Pedigree canned food (canned food is optional at Owner’s discretion.) Breeder highly recommends that the puppy have 24 hour access to food and water at least until over the age of 7 months.
6. All expenses regarding the puppy including shipping of the animal, proper care, veterinary fees, housing, and legal liabilities are the responsibility of the Owner at whose home the puppy will reside.
